Fabrikarbeiter John Q. Archibald ist zum Äusersten entschlossen! Sein Sohn Michael ist schwer erkrankt und benötigt dringend eine Herztransplantation. Er selbst kann sich die Operation finanziell nicht leisten und die Versicherung will nicht für die Kosten aufkommen. Eine scheinbar aussichtslose Situation. Als es seinem Sohn immer schlechter geht, sieht John Q. nur einen Ausweg: um Michaels Leben zu retten, beschließt er kurzerhand, sich in der Notaufnahme des Krankenhauses zu verschanzen und alle Anwesenden als Geiseln zu nehmen.
